what i mean is that you will fill in the space in the letter with the account number where the money will be transferred, the name of the bank and the address.

How are you finding it hard and what is the problem? just fill in the space and send it through e-mail with the bank email address i gave you, but if you cann't fill it forward the account number and address to me so that i will fill it for you.
